Below you will find our templates for the October 31st deadline, wherein we do everything for you except fill in the answers.   Copy and use verbatim  - 100% from the beginning to the end of every template - each template provided.

Prepare and send an email addressed to us - mpwcfoundation@gmail.com, between October 26th and October 31st of every year.   The subject of this email should be "EXAMP-2014-Section A Confirmation" (where the subject explains your 5-digit name, the grant year, and the title of what you are sending us).   You should copy and paste the below templates (emails and/or attachments) (nothing less, nothing more) and send them to us after inserting your own answers.  Be sure to conform to the following color-coding.

Things you should read, then possibly change to ENTER your own information, and then if changed, change the color coding to BLACK

Things you should read as they are instructive in nature, but you should delete or erase them (and close up the space) before sending them to us. (GREEN)

Things that ask questions or make statements that will remain unchanged on the input, but which will be followed by the below... (NAVY)

Responses to the above questions or statements from us, that respond to the above questions or statements. (BLACK)

Changes (this year's editings, deletions, additions) to the above that was otherwise THE SAME as you sent us last year.  Note that in these cases, after the grants have been paid, the changes are to be re-colored to match and become the ongoing response. (RED).

The template for the EMAIL that you should copy and paste is immediately below (the subjecct is: EXAMP-2014-Section A Confirmation)

To the Trustees of the MPWCF:

This email's format has been updated by the MPWCF on July 2, 2013.

This email is submitted in accordance with your web-page http://www.mpwcf.org/require-2.htm, Section A

The current email address of our organizations is example#1@isp.com.  Our current "contact person" at that address is Joe Jones and his email address is example#2@isp.com   The alternate to him is Tom Brown who has an email address of tbrown@isp.com.
It is our desire to continue in the MPWCF's grant program for the grant year 2014.
We have reviewed the 20 or more questions on the due Oct 31st webpage and submit the attached updated answers (using the color-coding to differentiate this year's changes, if any, from last year's answers).  The attachment is called "EXAMP-2014-20 Questions"    See below for the format you are to use to respond to in the 20 questions attachment.
We have re-read some additional details on your web-site page entitled http://www.mpwcf.org/some_details.htm and will follow its instructions and use THAT method for how to REPLY to any emails (especially the long and graphically detailed section named "Regarding EMAILs and REPLYs to emails" near the bottom of the page") sent by you to us.
We have already been working and currently continue to work on our responses to MPWCF's last year's  "Comments .. due by Jan 15" email and are considering how we can incorporate each such comment in our  "Due by Jan 31st input"
We acknowledge that we have both read and fully understood the implications on this year's and future grants described in the entire table at the top of subsequent changes
We acknowledge that the persons signing this paper below have either (1) prepared this email and/or the attached documents or (2) verified for completeness and accuracy this email and all of the attached documents.  We also acknowledge that the process necessary for continuity in the future has already been documented in records accessible to the entire organization in the event of a future absence of any one of us.

signed by (name, title/position) of the person writing this email

signed by (name, title/position) of any person(s) verifying any of the information included in any attachment (and, if more than one person verifying, which parts they take responsibility for verifying).

The following response to the 20 or more questions would probably be more EFFICIENTLY prepared if you had an ONGOING AMENDABLE (or editable) LIST of the answers to these questions and you submitted the most currently updated copy of it to us WITH MARKINGS (to differentiate) the current year's changes, if any, on this list.    Then use that list (again next year, as once again amended and updated) for the starting point for any future years.

The following is the template for you to copy for the ATTACHMENT  to the above email

This is EXAMP-2014-20 Questions.  The template itself has been updated by MPWCF on July 2, 2013.  Be sure to COPY and PASTE THIS ENTIRE (100%, including these instructions) TEMPLATE to a blank MS Word document as the first step (and starting point) for this year's input.  Then EDIT ONLY YOUR ANSWERS to update it for this year.  Be sure that YOUR responses show our questions EXACTLY AS THEY LOOK HERE.

The responses have been updated by (name of person responsible) and reviewed by (name of reviewer) and completed on (date)

1-Have you read and completely understood our entire Eligibility page and now send us this written statement affirming that you have met (and still meet) every one of the eight (8) requirements under the heading "Each new grantee organization MUST:"

Yes, we still meet each of the eight requirements except as detailed below.

2-Are you currently prepared to state the methods you use to ensure continuity and consistency of your programs through re-invigoration and eventual succession of your current management?  Your response may refer to an updated version of your annual attachment which will be due Jan 31st that states in much greater detail just what you are CURRENTLY doing to assure continuity

Yes, we have already started writing additional responses and editing previous responses and we will include them  in a detailed statement called "EXAMP-2014-Continuity" which we will send you with the rest of our January 31st input

After we received your grant in 2013, it was the completion of our year #9

4-Write a complete description of your purpose, mission, and/or goals (using your bylaws or conception documents as a starting point)

Blah, blah, blah (very completely do the above) blah blah blah.  You can start (for this and the below questions) with last year's response and edit it (--- that means adding, deleting, correcting, and otherwise making it MORE up-to-date than it was last year).

5-who, exactly, are your past (and your proposed future) beneficiaries and how do you maintain continuity with them, if indeed you do, in your own grant programs

Blah, blah, blah (very completely do the above) blah blah blah

12-Tell us the details regarding monetary compensation to all others (separately for employees, and volunteers, and officers, etc.)

Blah, blah, blah (very completely do the above) blah blah blah

13-Describe your financial status and past history (we strongly suggest you read financials-example)

We will illustrate this in our comparative and transparent financial statements which we will send to you by Jan 31st.  This statement will follow the format and provide all applicable information that you presented in your financials-example webpage (as well as including all of our own relevant disclosures).

14-Describe your current financial structure (again, read financials-example)

We will illustrate this in our comparative and transparent financial statements which we will send to you before Jan 31st.

15-Describe how you project future cash flow (both in and out), (once again, see financials-example)

Our budget for next year will be included in our comparative and transparent financial statements which we will send to you by Jan 31st.  We will also estimate  the various categories entering into our cash flow (detailed year by year and by individual descriptive category) for the next 5 or more years in a separate spreadsheet to be named "EXAMP-2014-Cashflow Projections"

16-Describe your past methods of (and future plans for) obtaining funding from sources other than us.

Blah, blah, (very completely do the above) blah blah - if quite long, we are attaching a statement called "EXAMP-2014-blah"

19-Describe your internal controls, both financial and other (see internal control),

We are already working on this year's most updated answers to a questionaire and it will be presented in a detailed attachment called "EXAMP-2014-internal control" using your internal control web-page as a starting point.  We will use the template on the due Jan 31st webpage as an aid to our answering this.

20-what, if anything, do you do to aid the mission of any other San Miguel charitable organization

Blah, blah, blah (very completely do the above) blah blah blah

21-what, if any, are the sections (or words) on our website that confuse you, or you feel contradict other instructions, or you have difficulty understanding as now written.

Rather than complain when MPWCF's next Comments letter(s) is received by us, we document on an attachment (called EXAMP-2014-problems and illustrations) all problems we have in understanding anything on the MPWCF website.  Each problem word or paragraph is reproduced on the attachment in a format as follows.  First, the exact quote of what we do not understand), followed by a specific, carefully worded description of our problem with each specific quotation.  This may be followed, where necessary, with a second (or third) direct quote if there are more contradictions or problems.  We believe this will enable the MPWCF to understand our problem and  clarify it for us (or correct it, if necessary) so that neither we nor any other grantee has that problem any more, nor any excuse for our not understanding what the MPWCF needs and wants from us.
